Medical Information
In order to protect children from common serious childhood illnesses, the State of Wisconsin has joined all other states in requiring a series of immunizations for entry into school.
Parents are required to notify the school of the child's immunization record upon entry into school. If your child has not completed the full series of the required vaccinations, please arrange for this as soon as possible. Vaccines can be obtained from your physician or at no cost from Public Health Madison & Dane County (266-4821).
School districts are required to assure compliance and report non-compliant students to the County Health Department and to the County District Attorney. Please be sure to tell us the dates of all your child's immunizations as they are given. We have no other valid source of information except the parent.
If for personal or religious reasons you do not want your child to receive the required vaccinations, you may complete a waiver form. Please contact your child's school office for this form.
The following links provide reliable medical information for you to look at. Please check the Immunication Requirements link to review the minimum required immunizations for each age/grade level. It is not a recommended immunization schedule for infants and preschoolers. For that schedule, contact your doctor or local health department.
