If you need a work permit, you may visit the high school office. Please take the following items:

Social Security Card (you must have the card as we must provide a copy of the card with the Child Labor Permit)

Letter from employer

Letter from parents** (see the end of this page for a sample format)

$10.00 fee

You must bring a birth certificate or baptismal certificate as proof of your age. The Wisconsin Department of Industry, Labor, and Human Relations does not accept driver’s license or identification cards as proof of age.
** Please insure that parents and the student employee include printed names and signatures.
You will be asked to provide information for the official form to include:

Type of work you will be doing
