Staff Development - Form A & Form B
Form A is used to establish two to three instructional or professional goals for the following school year (2009-2010). Once these goals are established, you must have them approved by your primary evaluator.
Form B is used to document the activities completed during the 2009-2010 school year to meet the approved goals from Form A submitted last spring.
