Medications at School
To ensure student safety, the Waunakee Community School District (WCSD) has clear guidelines for medication administration during the school day.
Students may take medication at school with written permission from a parent/guardian and, in some cases, a health care provider. This includes prescription medications, non-prescription medications, and over-the-counter (OTC) medications.
All medications require a completed WCSD Medication Administration Form. Please review the forms carefully to determine whether a parent/guardian signature is sufficient or if a health care provider’s signature is also required. Completed forms must be submitted to the school health office and reviewed by the School Nurse.
Important Information about Medications at School

- Non-FDA Approved Substances: These require a WCSD Prescription Medication Administration Form, signed by a health care provider.
- Emergency Medications: Parents/guardians must notify the School Nurse via email or phone if their student requires emergency medications (for asthma, allergies, seizures, or diabetes). The School Nurse will work with families to develop an administration plan.

- Students may carry and self-administer both prescription and OTC medications.
- Students may not share medication with others under any circumstances.
- A completed WCSD Over-the-Counter Medication Administration Form is required for OTC medications.
- A completed WCSD Prescription Medication Administration Form, signed by both a parent/guardian and a health care provider, is required for prescription medications.
- Medication Packaging & Verification:

- Parents/guardians may be asked to verify the medication quantity when dropping it off at the school health office.
- All forms required for medications can be found on the Student Services Form website.
